A Progress Report Form must be completed and sent to the Ministry of Community Services every quarter (March 31st, June 30th, September 30th and December 31st) until the project is complete. A Progress Report Form is required even if the project has not been started.

Upon project completion, the recipient will be required to submit a Final Report Form detailing project performance. The recipient is responsible for ensuring that the means to collect and provide data on project performance is in place.
